Who Qualifies

• You may be eligible to participate in this settlement if you meet the following criteria:

• You are a resident of the United States (settlement is nationwide)

What You May Receive

Eligible class members may receive one of the following forms of compensation:

• Ordinary Losses: Up to $2,500 in reimbursement for documented out-of-pocket losses reasonably related to the data breach
• Extraordinary Losses: Up to $10,000 in reimbursement for documented extraordinary losses related to the data breach
• Combined Maximum: Up to $12,500 in total reimbursements (ordinary and extraordinary losses combined)
• Cash Payment Alternative: A $100 cash payment if you do not wish to submit documentation of losses
• Credit Monitoring: Two years of three-bureau credit monitoring and identity theft protection services, available to all eligible claimants
