How to File a Claim

2. Complete the claim form in full, providing your personal information as requested and selecting the appropriate claim category (extraordinary losses, ordinary losses, or the $50 alternative cash payment).
3. Gather and attach required supporting documentation for any losses you are claiming — acceptable documents may include bank statements, phone bills, postage receipts, and gas receipts. If you are claiming the $50 alternative cash payment, documentation of losses may not be required.
